Sweat glands are an important part of the human body’s excretory system that play a role in maintaining balance. Sweat is a natural body fluid produced by the sweat glands in your skin.

Structure of Sweat Glands

The human body is estimated to have around 1.6 to 5 million sweat glands, and this number varies among individuals. Sweat glands are generally divided into two types: eccrine and apocrine glands.

1. Eccrine Glands

Eccrine glands are distributed throughout the skin, except in certain areas such as the lips, clitoris, penis, and labia minora. Their structure resembles a coiled tube and consists of two parts: the secretory portion and the duct.

Eccrine glands function from early life. From the age of 2–3 years, the total number remains constant throughout life. The highest density is found on the palms and soles (approximately 250–550 glands/cm²), and they respond to emotional and thermal stimuli.

The secretory portion lies deep within the dermis and contains two types of cells: clear cells that produce water and electrolytes, and dark cells that release macromolecules found in sweat.

Eccrine glands produce most of the sweat, especially the clear, watery type. Nevertheless, it is not pure water, as it contains small amounts of salt, protein, urea, and ammonia. Most eccrine glands are found on the palms, soles, and forehead.

2. Apocrine Glands

Unlike eccrine glands, apocrine glands are only found in specific areas such as the scalp, armpits, breasts, face, and perineum. These glands are larger and softer, and their structure is not coiled like eccrine glands.

After puberty, apocrine glands produce thicker secretions. This is why they are often associated with body odor. In fact, sweat itself is odorless, but when apocrine secretions mix with bacteria on the skin, they can produce an unpleasant smell.

Disorders of Sweat Glands

Disorders of sweat glands can be divided into three types: excessive sweating (hyperhidrosis), reduced sweating (hypohidrosis), and absence of sweating (anhidrosis).

1. Hyperhidrosis

Hyperhidrosis is a condition in which the body produces excessive amounts of sweat. Normally, the body sweats more during intense activity, exercise, hot weather, or stress. Nevertheless, hyperhidrosis can also indicate underlying health issues such as:

  • Diabetes

Secondary hyperhidrosis involves generalized or unilateral sweating due to underlying physiological conditions (fever, pregnancy, menopause), diseases (malignancy, infection, cardiovascular, endocrine/metabolic, neurological, or psychiatric disorders), or medication.

2. Hypohidrosis and Anhidrosis

If hyperhidrosis involves excessive sweating, hypohidrosis and anhidrosis refer to reduced or absent sweating. Anhidrosis can even be life-threatening due to hyperthermia.

Patients with these conditions often show symptoms such as dry skin (xerosis) and heat intolerance, which can lead to fatigue, weakness, dizziness, and difficulty breathing. Some conditions that may cause hypohidrosis and anhidrosis include:

  • Dehydration
  • Hypothyroidism (underactive thyroid)
  • Burns that damage sweat glands
  • Certain nerve disorders
  • Prolonged heat exposure
  • Exercising in hot seasons
  • Certain skin conditions
